Antibioprophylaxie chirurgicale : une évaluation des pratiques professionnelles - 30/03/08

Dans le cadre de la démarche d’accréditation de l’établissement, nous avons réalisé une enquête observationnelle des pratiques d’antibioprophylaxie chirurgicale. Les référentiels utilisés étaient les recommandations de la SFAR (Société Française d’Anesthésie Réanimation) et un protocole local, établi en 1996. Nous avons évalué la conformité des différents paramètres de l’antibioprophylaxie, et repéré les points faibles de l’organisation : horaire d’injection (conformité de 66 %), et durée d’administration (78,5 %). L’analyse des résultats a mis en avant la nécessité de revoir le protocole local, ainsi qu’un problème de responsabilité de la prescription de l’antibioprophylaxie pouvant conduire à des administrations inutilement prolongées. Ce travail a été l’occasion d’engager une réflexion sur les facteurs influençant l’adhésion des praticiens aux recommandations. Les contraintes externes sont apparues prépondérantes. Des axes d’amélioration ont été mis en place à l’issue de ce travail : révision du protocole, rédaction d’une procédure.
Within the framework of the step of accreditation of our hospital, we carried out an observational investigation of the practices of surgical antimicrobial prophylaxis. The reference frames used were the recommendations of the SFAR (French Society of Anaesthesia and Reanimation) and a local protocol drawn up in 1996.
We evaluated the conformity of the various parameters of the antimicrobial prophylaxis, and located the weak points of the organization: schedule of injection (66% of conformity), and duration of administration (78.5%). The analysis of the results proposed the need for re-examining the local protocol, as well as a problem of responsibility for the antimicrobial prophylaxis prescription which can lead to unnecessarily prolonged administrations. This work gave us the opportunity to engage a reflexion on the factors influencing the adhesion of the prescribers to the recommendations. The external constraints appeared dominating. Axes of improvement were set up at the end of this work like the revision of the protocol and a procedure drafting.
